Physiotherapy that works towards your NDIS goals

Every NDIS plan is built around goals — and funded supports are meant to build your capacity to reach them. That's precisely how we practise. Physiotherapy here isn't a set of exercises for their own sake: it's targeted work on the strength, movement, balance and confidence that sit underneath your goals, whether that's showering independently, transferring without a hoist, walking to the shops, holding down a job, or joining a community sport.

Because we're fully mobile, therapy happens in the environments the goals belong to. If the goal is safer transfers, we work with your actual bed and bathroom. If it's community access, sessions can happen on the actual route, in the actual park, at the actual gym.

Who we support

We work with NDIS participants aged 18 to 64 who are self-managed or plan-managed.

If your plan is agency-managed (NDIA-managed), we aren't able to provide services at this stage, as those supports must come from registered providers — your support coordinator can help you find one. How physiotherapy can support your plan

Mobility, walking and transfers — including hoist-free options where safe and realistic · strength, endurance and motor skills · falls prevention and safety at home · pain management strategies you can use daily · assistive technology and equipment recommendations, trialled in your real environment · training for your support workers so therapy continues between sessions · graded programs for community access and social participation.

What to expect

Initial assessment. We begin with your goals and your story, then complete a functional assessment — how you move, transfer, walk and manage daily tasks now. Allow 1 to 1.5 hours. You receive a written therapy plan that links each piece of therapy to a plan goal, with baseline outcome measures recorded so progress is provable, not just felt.

Ongoing therapy. Regular sessions at home or in the community, a program that's progressed as you improve, and check-ins with the rest of your team — support workers, OT, coordinator — so therapy pulls in one direction.

Reports when you need them. Plan reviews run on evidence. We provide a progress report prior to every plan review — and any time on request — written in NDIS language, linked to your goals, with outcome measures. The kind of documentation that helps the planner see what the funding achieved and what's still needed.

Funding, pricing and agreements

Physiotherapy is usually funded from the Capacity Building — Improved Daily Living part of an NDIS plan. Our rates align with the current NDIS Pricing Arrangements and Price Limits, and travel is charged as set out in the Pricing Arrangements.

Before therapy starts, you'll receive a written service agreement covering session rates, travel, report writing and cancellation terms — so you know exactly what will be invoiced and there are no surprises for you or your plan manager.

For support coordinators and plan managers

Referrals to admin@coastalcommunityphysio.com.au — helpful inclusions are the participant's goals, plan management type, funding available, and any risks we should plan for (behavioural, environmental, manual handling, animals at the property).

From us you can expect: referrals acknowledged within 1 business day; service agreements returned within 3 business days; progress reports prior to plan reviews and on request; session notes on request; prompt flagging if funding is tracking short of the therapy plan; and invoices emailed directly to the plan manager.

Yes — program handover to support workers is one of the most effective things we do, because it turns one session a week into daily practice.

We ask for a minimum of 2 business days' notice, consistent with the current NDIS rules for therapy supports. Full terms are in your service agreement before we start.
